NWH RN CASE MANAGER

The RN Case Manager (CM) facilitates the coordination of care for those patients
admitted to Medical, Surgical, Pediatric or Obstetrical Services and as needed
for hospital outpatient areas including but not limited to the Emergency
Department and Henderson Surgical Center.  The Case Manager will be responsible
for monitoring the utilization of services and quality of care in order to
ensure the delivery of high quality, cost effective patient care. She/he will
develop and implement case management plans for those patients identified as
requiring post discharge services utilizing nursing process. The Case Manager
will serve as a resource for physicians, hospital staff and patients and their
families in the areas of managed care, community resources, and problem solving.
The role requires the ability to offer creative, problem solving techniques
using sound and prudent clinical judgment. The Case Manager continually updates
and educates him/herself in the areas relating to case management, managed care
and other pertinent areas. 

The RN Case Manager consults, and collaborates in activities related to the
establishment of appropriate level and venue of care for patients.  He/she
concurrently reviews and monitors patient records and participates in direct and
indirect communication with caregivers, to ensure continued appropriate level of
care, utilization of hospital and medical resources, and participates in quality
and risk management case finding activities.  The RN Case Manager performs,
consults, and collaborates in activities related to the safe, effective and
efficient discharge or disposition of patients.

Every other Saturday and every Sunday - 8 am-4:30 pm

MA lic required. Minimum 10 years RN case mgmt experience in an acute care hospital experience required. Understanding of utilization management, third party payor and insurance, knowledge interqual criteria helpful.  Discharge planning exp required.
